NHS Turns 75: Cupcakes, care, and community connections - West Mendip celebrates with a road trip

Tomorrow (Wednesday, 5th July), NHS staff from the West Mendip Primary Care Network (PCN) are hitting the road, heading to Wells, Glastonbury, and Street.
They'll be bearing gifts - 75th birthday cupcakes, courtesy of Burns the Bread, and hot-off-the-press news about your local NHS services.
Burns the Bread are pitching in for the festivities by whipping up 75 carrot cupcakes, which the NHS crew will dole out to lucky locals and businesses in the hearts of Wells, Glastonbury, and Street.
The West Mendip PCN, a super team of all five GP practices in the area - namely Wells Health Centre, Wells City Practice, Glastonbury Health Centre, Glastonbury Surgery, and Vine Surgery - is making it all happen.
The PCN maintains a dedicated Integrated Health and Wellbeing Hub that supports patients throughout the towns and villages of West Mendip, catering to a population of over 50,000 individuals.
The Hub goes beyond your average GP practice. Its team includes Care Navigators, Care Co-ordinators, Health Connectors (real-life health coaches), a GP, complex care nurse, clinical pharmacists, an occupational therapist, and a hospital discharge reviewer.
With a motto of "No problem too small", the Hub provides help with everything from finding a local book club to getting a lift to the shops, support for carers, lifestyle tips, financial aid, digital support, and even assistance with addictions.
The team is also there for people navigating the complexities of health issues such as dementia, cancer, and frailty. They make sure these individuals receive the right care, support, and treatment, often working with a whole orchestra of healthcare professionals.
Dr Mike Pearce, a GP at Wells City Practice and the clinical lead for the PCN, stated: "To commemorate the NHS's 75th birthday, we wanted to tour our local communities to express our profound gratitude for their continual support and inform them of some primary care developments.
"Through our network of 5 practices and a central Hub, we can offer enhanced, personalised, and proactive support to local patients, seamlessly integrated with secondary care, social care, and the voluntary sector."
